Please check in with our receptionist when you arrive. She will obtain all of the necessary documentation to file your insurance claim successfully.
In order to examine the retina, your eyes will be dilated. If further tests are needed, they will probably be performed immediately.
Your active participation and cooperation are important to positive long-term results. We will provide the information you need to make your visit as helpful and informative as possible.

Examination
The initial evaluation will
be comprehensive. We will do our best to complete
all necessary tests, explain
results and make recommendations at that time. Because
both eyes will be dilated, we strongly advise against
driving. We suggest bringing a friend or relative
to drive you home after each visit. Please
plan at least two hours for your appointment and
bring the following:
|
• |
If necessary, please arrange for a
referral with your HMO primary care physician prior
to the date of the appointment. The referral may
be called in to 314.727.6711 between 8:30 am and
5:00 pm. It can also be faxed to 314.727.0705, at
least 24 hours before the appointment. |
• |
A current address and phone number
for your medical doctor and eye doctor. |
• |
Your co-pay, if required, and all
insurance cards. |
• |
A list and/or actual containers of
all the medicines you currently take, prescription
and over-the-counter. |
• |
Sunglasses |
 |
 |
|
Before you leave our office, every
effort will be made to explain the treatment alternatives
available for your condition. We are happy to include
family members in this discussion. Please feel free
to ask questions at any time.
